Post-Training Staff Questionnaire

The post-training staff questionnaire is to be completed after finishing the entire one-year AF-CBT training program, including all consultations. Here you can update any changes in your contact information and provide us with some feedback about your training experience, which will aid us in our efforts to constantly improve the program. Filling it out is a part of the training and is required in order to obtain a completion form or to move forward with certification. (This takes about ten minutes to complete)
